Face à l’explosion du prédiabète et du diabète de type 2, les régimes alimentaires ne sont plus de simples recommandations générales : ils deviennent de véritables outils thérapeutiques
Mais lequel recommander, en pratique ?
L’efficacité du régime peut se mesurer notamment par la baisse de l’HbA1c — un marqueur-clé du contrôle glycémique.
L’HbA1c reflète la glycémie moyenne sur les 2 à 3 derniers mois. Plus elle est élevée, plus le risque de complications (rétinopathie, neuropathie, maladies cardiovasculaires) augmente.

Comparatif des régimes les plus étudiés
🔹 Régime pauvre en glucides (Low-Carb)
- Définition : réduction modérée des glucides (<130 g/j)
- Effet sur l’HbA1c : −0,28 % à −0,44 %
- Remarques : efficace à court terme, mais l’effet diminue sans accompagnement durable.
🔹 Régime cétogène
- Définition : très faible en glucides (<50 g/j), riche en lipides
- Effet sur l’HbA1c : −0,38 % à −1,1 %
- Remarques : effet rapide et marqué, nécessite un suivi médical, pas toujours adapté à long terme.
🔹 Régime méditerranéen
- Définition : riche en légumes, fruits, huile d’olive, légumineuses, poissons
- Effet sur l’HbA1c : −0,3 % à −0,5 %
- Remarques : excellent compromis entre efficacité, adhésion et bénéfices cardiovasculaires.
En pratique :
- Le meilleur régime est celui que le patient peut suivre à long terme, sans frustration, avec une amélioration durable de son équilibre glycémique et de sa qualité de vie.
- L’accompagnement nutritionnel personnalisé est la clé d’une adhésion durable.
- Adapter l’alimentation au profil du patient : âge, traitement, IMC, comorbidités, adhésion potentielle.
- Prévoir un suivi régulier : HbA1c, poids, périmètre abdominal, lipides, bilan nutritionnel.
